Abstract
Premature infants exposed to second hand smoke experience disparate respiratory health outcomes compared to those who are not. These disparities propose significant problems to the health system and can bring substantial morbidity and mortality and life-long reliance on special healthcare needs. Caregivers of these infants experience increased stress, which serves as a risk factor for engaging in smoking and stress avoidance serves as a barrier to smoking cessation. To address these overlooked variables, the ""Room to Breathe"" program framework is proposed using the PRECEDE-PROCEED model to assess the needs of the community and address the variables causing disparities of respiratory health for this population. The ""Room to Breathe"" program will offer the evidence-based tobacco cessation resources of nicotine replacement therapy, motivational interviewing, and behavioral support groups to assist caregivers of premature infants born with bronchopulmonary dysplasia who self-report smoking in the home. A unique opportunity exists in contact frequency of this population with pediatric hospitals that lends itself to improved health outcomes; the established resources at pediatric hospitals including social workers, care coordinators, RN case managers, and established pulmonary service lines, where many children affected by prematurity are cared for throughout their childhood. Included in the report is a review of the literature, socioeconomic considerations, evidence based-smoking cessation strategies, application of the PRECEDE-PROCEED model, program framework, program evaluation outlines, and conclusions and recommendations for implementation.
